1.  How does your intake process work?

When an inquiry is made into one of our programs, a packet is sent out.  Included in this packet is an intake application.  People with and without disabilities are able to attend.  After receiving the intake application and necessary correspondence has taken place, it will be determined if the client will require a personal aide.  If required, the personal aide will need to attend all functions in which the client participates that is affiliated with Natural Settings CBI.  At this time, the group is selected for the client:  1. Young teen (11-14), 2.  Teen/Adult (15 - 20 years), Adult (age 21 and over).

2.  Who will be present at your programs?

A licensed occupational therapist or special education teacher will be present at all programs.  There will be auxiliary staff offering a 2:1 client/staff ratio.  Additional staff such as nurse, speech therapist, sign interpreter will be present as the need arises.  A presenter (teacher or craftsman) will also be present at the Career Exploration Workshops.  A person from the 4H extension Master Gardener Program will be available as needed, and an assistant certified in ServSafe practices will be present at the Bakery workshops.  Additional staff will be present dependent on number of clients served.  Maximum clients in a given program is 20 with a professional staff person on site for every 10 clients.

3.  How can I contact Natural Transitions Inc when on a community outing?

Natural Transitions Inc has a 24 hour answering service who knows how to contact us on all outings.  We also carry client's contact information with us so that if there is an emergency, contact persons will be called.

4.  How do I know how I am performing?

Daily notes will be sent home in client's own communication book.  Also, a monthly progress note or end-of-project notes will be sent home.  It is Natural Transitions Inc mission to increase the client's job market potential.

 5.  How does Farmer's Market day work?

When in the 2-week program, we use the Ann Arbor farmer's market on both Wednesdays, all day. Produce will be labeled the previous day and will be delivered to sell on market day.  Clients are entitled to what they sell from their gardens.  Money can be set up in a trust fund if guardians so wish.  We will also be off-site for the second Wednesday lunch to experience ordering in a restaurant.

 6.  Where will you be off-site?

In inclement weather in the mornings except Wednesday, we will locate our seminar in the Meri Lou Recreation Center next to County Farm Project Grow gardens.

The first Friday of the program we will have an option of attending Secretary of State office for certification (Food Handler) or attending an available advocacy site:  Ann Arbor Center for Independent Living, YMCA, Ann Arbor Library, Michigan Rehab Services are examples.

On Monday afternoon of the seminar, we will vist MITC (Michigan Information Technology Center) to set up our portfolios (if large enough number of clients in seminar) or Merri Lou Recreational Center (if small # clients present).

The goal is to enrich clients experience with the job market, life skills, and social integration.

 7.   What about financial help?

We offer a scholarship if funding can not be found by other money streams.  A IRS 1040 must be submitted by April 15, 2010 for consideration.   We also offer an heirloom seed fund-raiser to help defray tuition costs.  Orders must be received by May 1, 2010 so orders can be made and delivered in an adequate time frame. Order form is located on this website:  Scholarship fund-raiser order for:   heirloom seed fundraiser
